Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare providers
- Administer medications accurately following prescribed instructions while maintaining detailed medical documentation
- Support patients with specialized needs including dementia care, Alzheimer’s care, memory care, and working with individuals with developmental disabilities or disabilities
- Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and meal preparation
- Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical records securely in compliance with HIPAA regulations
- Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation
Requirements
- Proven experience in home health care, nursing homes, assisted living facilities or long-term care environments
- Knowledge of medical terminology and procedures related to patient care
- Valid CPR certification and training in catheter care, hospice care, dementia care, and medication administration preferred
- Ability to work independently while following healthcare plans and protocols precisely
- Strong observational skills for patient safety monitoring including assessing vital signs and recognizing changes in condition
- Experience working with individuals with disabilities or developmental challenges is highly valued
- Familiarity with EMR/EHR systems and medical documentation standards
- Compassionate attitude combined with excellent communication skills and professionalism
- Ability to lift or transfer patients safely using equipment like Hoyer lifts when necessary
