Dove Healthcare- South Eau Claire is seeking an LTE nurse case manager to add to our team.

RN Nurse Case Manager Responsibilities:
- coordinate and oversee palliative care services to promote comfort, symptom management, and quality of life
- support and collaborate with rehabilitation services to assist residents in achieving optimal functional outcomes
- provide compassionate care that addresses physical, emotional and spiritual needs of patients and their families
- supervise the day-to-day activities of assigned residents and employees
- coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence
- review and audit compliance with the patient's plan of care and changing needs
- ensure appropriate staffing patterns are in place
- evaluate employee duties and needs
- hold employees accountable for following policies and procedures
- provide a safe, comfortable, and home-like environment
- on-call rotation
- other duties as assigned
Required Qualifications:
- current licensure in the State of Wisconsin as a registered nurse
- degree from an accredited school of nursing
- previous experience in related field
- ability to prioritize duties in a constantly changing environment
- effective management skills
- customer service oriented with patients, employees, families, and other agencies
- good problem solving skills
- excellent time management skills
- ability to accept and give directions
- work well in a team environment
- a desire to make a difference in people's lives
