SUMMARY
: This is responsible professional work in support of Senior Services wellness-based programs, activities, and events. The position assists the Senior Services Program Manager in the development, implementation, execution and evaluation of a diversified program of senior wellness activities at Senior Services. Major job functions include coordinating non-emergency medical transportation services, conducting participant assessments, scheduling and managing medical appointments, organizing wellness initiatives, and maintaining detailed and accurate participation records. Work is performed under the immediate supervision of the Senior Services Program Manager, and the employee has considerable independence to manage and complete program assignments.ESSENTIAL DUTIES AND RESPONSIBILITIES
Duties include but are not limited to:

Coordination of non-emergency medical transportation, to include working with a variety of medical professionals and participants to schedule/reschedule appointments
·
Conduct needs assessments for non-emergency medical transportation
·
Assess outdoor pick-up areas around participant residences for safety hazards, including driveway conditions, traffic exposure, and wheelchair loading/unloading accessibility.

Assisting in conducting needs assessments to determine senior programming needs and interests
·
Assisting in the development, implementation, promotion, and evaluation of programs offered
·
Assisting in promoting the benefits of an active and healthy lifestyle through interactive programs and written materials
·
Assisting in the development and promotion of chronic disease prevention and education programs
·
Working with center staff to increase the participation of seniors from the community in health and wellness activities
·
Work in conjunction with Program Assistant to provide information on current health issues affecting older adults including nutrition, chronic diseases, cognitive and mental health issues, and physical and functional changes.

Performing public relations activities to include speaking to civic/community groups
·
Maintaining the medical equipment loan closet
·
Maintaining accurate records and files and preparing periodic and special reports
·
Maintaining program supplies and reorders as necessary
·
Working closely with other staff within Senior Services to provide and coordinate programs and services for seniors, to include meal delivery to homebound clients
·
Performing other duties as assigned
QUALIFICATION REQUIREMENTS:

MINIMUM EDUCATION and/or EXPERIENCE:

Ability to plan, coordinate, and manage logistics for medical transportation across multiple schedules

Three years of professional related work experience
·
An equivalent combination of education and experience may be considered
ELIGIBILITY REQUIREMENTS:

Must be 18 years of age or older
CERTIFICATES, LICENSES, REGISTRATIONS:
·
Possess and maintain a valid Georgia driver’s license (Class C) and a satisfactory motor vehicle record (MVR)
·
First Aid/ CPR Certification within six months of hire
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of or ability to learn county and department operations, ordinances, policies, procedures, terminology, and materials as they relate to assignment
