Summary of Essential Functions for the Position:
Committed to Caring, the Managed Care Coordinator supports the delivery of services by coordinating patient care needs with managed care requirements, ensuring clinical teams and documentation are aligned with authorization requirements and the patient’s overall plan of care. This role bridges clinical and administrative functions, in helping to optimize patient outcomes, ensuring compliance with plans of care, supporting timely documentation and adhering to payer requirements.
Qualifications / Licensure / Certification / Knowledge / Skills / Abilities / Shift / Compensation:
- One (1) to two (2) years of experience in healthcare, home health care preferred
- Paraprofessional level or above credentials (LVN, PTA, COTA) preferred
- Ability to be well organized, self-motivated, self-directed, and detail orientated
- Ability to work independently with minimal supervision
- Ability to work within a team environment and with deadlines
- Strong communication skills, both oral and written with excellent interpersonal skills
- Must demonstrate good customer relations skills and a commitment to providing quality service
- Acceptance and ability to demonstrate and support the core values and goals of Agency
- Proficient with computers and Microsoft Word, Excel and Outlook software
- Proficient with using the Agency EHR system and data reports to review, analyze and mitigate issues
- Valid driver’s license, automobile liability insurance and reliable transportation required
Educational Requirements:
- High school diploma or GED. Completion of post-secondary education or training in a healthcare field preferred. Paraprofessional-level credentials (e.g., LVN, PTA, COTA) preferred.
Working Conditions & Physical Requirements:
- Work environment can be stressful due to deadlines, multiple tasking and dealing with compliance of law, rules and regulations to ensure optimal patient outcomes. The position requires visual acuity and dexterity, sitting, standing, some pushing, pulling and lifting up to 25 pounds. Auto related accidents possible.
Shift:
- Full-Time | Monday-Friday (08:30AM-05:00PM)
