Job Summary
We are seeking a highly skilled and compassionate Advanced Practice Provider (APP) specializing in Primary Care to join our healthcare team. The ideal candidate will be responsible for delivering comprehensive patient care across diverse clinical settings, including outpatient clinics, inpatient units, and post-acute care environments. This role requires extensive experience in medical documentation, patient assessment, and a broad knowledge of medical procedures and specialties such as internal medicine, geriatrics, pediatrics, behavioral health, and emergency medicine. The APP will collaborate with physicians, nurses, and other healthcare professionals to provide high-quality, patient-centered care while adhering to HIPAA regulations and clinical standards. Experience with EMR/EHR systems like Epic, Cerner, or eClinicalWorks is essential.
Responsibilities
- Conduct thorough physical examinations and assess patient health status across various age groups including pediatrics, adults, and geriatrics.
- Manage acute and chronic conditions utilizing advanced clinical skills such as suturing, IV infusion, catheterization, tube feeding, and sterile processing techniques.
- Perform diagnostic evaluations including laboratory specimen collection/processing, medical imaging interpretation, sonography, and diagnostic evaluation procedures.
- Administer medications and injections accurately while monitoring for adverse reactions; manage airway using advanced airway management techniques.
- Provide specialized treatments such as Botulinum toxin therapy, dermal fillers, esthetic laser treatments, gastric lavage, spinal taps, and ventilator management.
- Document all patient interactions thoroughly using EMR/EHR systems like Epic or Cerner; ensure compliance with HIPAA standards.
- Coordinate care plans including discharge planning, case management, utilization review, and post-acute care planning for diverse patient populations including those with Alzheimer's or dementia.
- Engage in health coaching and patient education to promote wellness and disease prevention strategies tailored to individual needs.
- Collaborate with multidisciplinary teams in settings such as Level I/II trauma centers, NICU/PICU units, hospital medicine wards, outpatient clinics, urgent care centers, hospice & palliative medicine programs.
- Stay current on clinical guidelines related to infectious disease care, pain management—including acute pain—and occupational health protocols.
Qualifications
- Master's degree or higher in Nursing (NP), Physician Assistant Studies (PA), or equivalent advanced practice credentialing required.
- Valid state licensure as an Advanced Practice Provider with certification in primary care or relevant specialty areas.
- Proven experience in a variety of clinical environments such as hospital medicine, emergency room/urgent care settings, nursing homes or assisted living facilities.
- Extensive knowledge of medical terminology (ICD-9/10 coding), CPT coding procedures for billing purposes.
- Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with telehealth platforms is preferred.
- Strong skills in patient assessment and diagnosis across multiple specialties including internal medicine, geriatrics, pediatrics (including neonatal care), behavioral health and more.
- Ability to perform invasive procedures such as venipuncture, phlebotomy, catheter care; experience administering injections is essential.
- Knowledge of infection control practices and aseptic techniques; experience working with patients with disabilities or developmental disorders is a plus.
- Excellent communication skills with the ability to educate patients effectively on treatment plans and health maintenance strategies.
