What are the responsibilities and job description for the Licensed Physical Therapist Wellness Consultant (PRN) position at Divinity Wellness?
Looking for a motivated, engaged , compassionate, licensed Physical Therapist for contract work within the Escondido and Ramona California areas.
This independent contract position works directly with individuals in their congregate living homes. As the lead physical therapist wellness consultant, the therapist will provide consults and care oversight for residents with a focus on providing patient care focused on functional status, activities of daily living, independent living skills, fine motor coordination, home safety recommendations, adaptive equipment training, strengthening and maintaining general strength and mobility. The Physical therapist will develop the consultation plan and collaborate with the care team to ensure that the plan of care is followed, performing interventions aimed at improving and enhancing the patient's wellbeing, and monitoring the patient’s progress. The successful candidate will also collaborate with supported living center staff ( nurse, direct support professionals, house administrator) to enhance the physical, mental, and social well-being for patients.
