What are the responsibilities and job description for the Licensed Occupational Therapist Wellness Consultant position at Divinity Wellness?
Looking for a motivated, engaged , compassionate, licensed Occupational Therapist for PRN work within the Escondido and Ramona California areas.
This independent contract position works directly with individuals in their congregate living homes. As the lead Occupational therapist Wellness consultant , the therapist will provide consultation and care oversight with a focus on functional status, activities of daily living, independent living skills, fine motor coordination, home safety recommendations, adaptive equipment training, strengthening and maintaining general strength and mobility. The Occupational therapist wellness consultant will develop the plan of care and collaborate with the care team to ensure that the treatment plan is followed, performing interventions aimed at improving and enhancing the patient's wellbeing, and monitoring the patient’s progress. The successful candidate will also collaborate with supported living center staff ( nurse, direct support professionals, house administrator) to enhance the physical, mental, and social well-being for patients.
