Divine House Inc. is accepting applications for a full-time Program Director supervisory position for the Big Stone County area which will office out of Ortonville Mn.
Competitive wages based on education and experience.
Starting salary $50,000-$60,000 annually.
Qualifications:
At minimum you must meet the Designated Coordinator education training and work experience requirements per 245D.081 Subd. 2 (b) clause 1-5.
A Designated Coordinator must have the skills and ability necessary to develop effective plans and to design and use data systems to measure effectiveness of services and supports.
PRINCIPLE RESPONSIBILITIES FOR THE DESIGNATED COORDINATOR:
To maintain a current understanding of the licensing requirement’s sufficient to ensure compliance.
To provide supervision, assistance and training necessary to ensure the health, safety and protection of rights of each person you oversee.
To provide coordination of service delivery and evaluation of services and must provide supervision, support and evaluation of activities that include:
1.) Oversight of the license holder’s responsibilities assigned in the persons
coordinated services and support plan and the coordinated service and
support plan addendum
2.) Taking the action necessary to facilitate the accomplishment of the
outcomes according to the requirements of 245D.07 (service planning
and delivery)
3.) Providing instruction and assistance to direct support staff implementing
the coordinated service and support plan and the service outcomes,
including direct observation of service delivery sufficient to assess
staff competency
4.) Providing an evaluation of effectiveness of service delivery,
methodologies, and progress on the person’s outcomes based on the
measurable and observable criteria for identifying when a desired
outcome has been achieved according to the requirements in section
245D.07
Program Director Administrative tasks:
1.)To oversee, report and/or correct building and equipment deterioration, safety hazards, and unsanitary conditions.
2.)To communicate and coordinate services with the person and their expanded support team to ensure continuity of care.
3.)Have knowledge, implement and ensure compliance of company policies
4.)Recruit, hire, and orientate new employees as well as maintain current personnel files for those in designated areas or as instructed by the company.
5.)To communicate and work in cooperation with all departments within the
company
6.)Complete a “Basic things to check” per company requirements, and
follow through with any discrepancies discovered
7.)Provide adequate staff training and conduct regular staff meetings
to maintain the operational needs of each site as assigned
8.)To maintain the proper documents in the records
9.)To follow administration instruction and implement all other duties
assigned
10.)Will have awareness of and implement the duties assigned in the
position descriptions of subordinate positions when necessary
