ResponsibilitiesPatient Care (Field):
- Provide skilled nursing visits in patients’ homes
- Perform assessments and monitor patient conditions
- Administer medications, treatments, and wound care
- Communicate updates with physicians and care team
Office Support / QA:
- Review and ensure timely completion of visit documentation
- Assist with EVV and EMR (Axxess) compliance
- Support scheduling and care coordination
- Participate in chart audits and quality assurance activities
- Assist with onboarding support for aides and field staff
Ideal Candidate
- LPN who enjoys both patient care and organization/coordination
- Detail-oriented and comfortable with documentation
- Interested in career growth into leadership
- Reliable, professional, and team-oriented
Requirements
- Active LPN license in Ohio
- Minimum 1 year experience (home health preferred)
- Reliable transportation
- EMR experience (Axxess a plus)
