What are the responsibilities and job description for the Care Manager position at District Three Governmental Cooperative?
DISTRICT THREE GOVERNMENTAL COOPERATIVE will be accepting resumes/applications for a Full-Time Care Manager at our Galax office location.
JOB SUMMARY: Under the immediate supervision of the In-Home Services Director, the employee shall provide outreach services, information and assistance to eligible clients. The Care Manager shall also be a contact person for District Three Governmental Cooperative in the field.
The starting pay for this position is $20.62 per hour (w/Competitive Benefits Package) and work week of 37.5 hours.
JOB QUALIFICATIONS: The Care Manager shall have received a Bachelor’s Degree in Social Work or a related field and two or more years relevant work experience. Proficient computer skills are required. Must hold a valid driver’s license and have "zero points or points" driving record. Experience working with the aging population preferred. A background check and substance abuse testing required.
