General Summary
Under the supervision of the Community Health Director, coordinates the grant-funded Rural Region 3 Home Visiting Local Leadership Group (LLG). This position oversees minimum program requirements and has a continuous quality improvement (CQI) focus on building a coordinated, high-quality, family-focused home visiting system. Builds relationships with parents and members of the LLG, facilitates and organizes membership and CQI meetings, implements projects and other deliverables. Some travel required.
Essential Functions
· Communicates with community partners and agencies to promote program services and best-practices.
· Create coordinated systems change to improve outcomes for children and families in the community.
· Participates in local, regional, and statewide meetings and leads local membership and CQI meetings.
· Assists in identification of needs and connecting individuals and families to community services addressing the Social Determinants of Health.
· Assurance to policy and program requirements in close communication with the Community Health Director.
· Prepares for and participates in program reviews conducted by Michigan Department of Health and Human Services and other regulatory bodies, including preparation and fidelity to accreditation standards.
· Assures that program staff are aware of and follow policies laid out by regulatory bodies and District Health Department No. 2 policy.
· Participates in Continuous Quality Improvement and Performance Management activities.
· Maintains client records and privacy per HIPAA/Confidentiality policies.
· Requires flexibility as the needs of the division change with programmatic and funding requirements.
· Other duties as assigned.

Education/Training
· Possession of a degree in nursing, social work, or equivalent field with current credentials/licensure to practice in the State of Michigan is required.
· Must have knowledge of social determinants of health and their impact on the community; and be able to work with community groups for solutions.
· Knowledge of home visiting services through evidence-based models.
· Ability to provide services to vulnerable populations in a culturally competent and non-judgmental manner.
Other Requirements
· Successful completion of BLS certification is a condition of continued employment.
· Valid Michigan driver’s license: or the ability to have personal transportation provided.
· Initial/annual training pertaining to MI-OSHA’s Blood Borne Pathogen Standard is a condition of continued employment.
· Computer proficiency with Microsoft Office 365 and Electronic Health Records.
