Position Summary

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement appropriate treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health services as needed, including supervisory visits, to ensure optimal patient outcomes
• Stay up-to-date with current literature and research regarding physical therapy modalities, treatment protocols, and clinical trends

Qualifications:

• A degree in Physical Therapy from an accredited program• A Florida state PT license and current CPR/AED certification• Experience in outpatient and/or home health setting preferred• Ability to work independently and as part of a team• Strong verbal and written communication skills• Possess solid problem-solving skills and critical thinking skills• Ability to work with a diverse patient population
