Position Summary
The Registered Nurse (RN) - Home Health is responsible for providing skilled nursing care to patients in the comfort of their homes. The RN - Home Health works in collaboration with the interdisciplinary team, physician, and family to develop a comprehensive plan of care to meet the patient's needs. As a full-time position, the RN - Home Health will be called upon to work on an as-needed basis, based on the needs of the organization. This is a full-time position. The RN - Home Health will be scheduled to work from Tuesday-Saturday weekly.

Essential Duties And Responsibilities
- Ensure patient safety and provide high-quality nursing care
- Assess the patient's condition and implement the care plan in collaboration with the interdisciplinary team
- Provide wound care, medication management, IV therapy, and other skilled nursing services
- Educate the patient and caregiver on disease management, prevention of complications, and self-care
- Collaborate with physicians and other healthcare professionals to develop and implement the plan of care
- Complete documentation of patient care in an accurate and timely manner
- Participate in quality assurance and performance improvement activities
- Attend and participate in interdisciplinary team meetings, as needed
- Active Florida RN license
- At least 1 year of nursing experience, with Home Health experience preferred
- Ability to work independently, with minimal supervision
- Strong clinical and critical thinking skills
- Excellent communication and interpersonal skills
- Reliable transportation
- Ability to work on an as-needed basis
