Job Title: PRN MSW Home Health Social Worker
Location: Venice, North Port
POSITION SUMMARY:
- Assess patient's emotional, environmental, social and financial needs, and develop appropriate care plans.
- Coordinate patient care with other healthcare professionals, such as physicians, nurses and therapists.
- Monitor and evaluate patients' progress, and adjust care plans as necessary.
- Educate patients and caregivers about resources and services available in the community.
- Provide counseling and emotional support to patients and families.
- Maintain thorough documentation of patient care in electronic medical records, as well as paper files.
Qualifications:
- Master's degree in Social Work from a school accredited by the Council on Social Work Education.
- Active and valid state license as a Social Worker.
- Minimum of one year of experience in the field of healthcare, home health or hospice preferred.
- Knowledge of community resources, and experience in navigating and coordinating care with local agencies.
- Excellent communication skills, both verbal and written.
- Ability to work independently, and to manage and prioritize multiple tasks in a fast-paced environment.
- Must have a reliable vehicle, valid driver's license, and auto insurance.
