What You’ll Do:
As a Registered Nurse at DSSW, you’ll play a vital role in supporting individuals in home and community-based waiver programs. Your responsibilities will include:
- Conducting initial assessments for clients transitioning off the CLASS waiting list
- Performing post-hospital assessments to ensure continuity of care
- Managing and completing annual renewal assessments
- Collaborating with a compassionate team to ensure the highest quality of care
- Current RN license in the State of Texas
- Minimum 1 year of home care nursing experience
- Strong typing skills (35 WPM with 80% accuracy) and Excel proficiency
- A reliable vehicle, valid auto insurance, and willingness to travel throughout Longview/Henderson
