About the Position:
As a Clinical Care Coordinator, you will be at the forefront of educating and assisting patients with their healthcare needs. Your responsibilities encompass a diverse range of tasks, with a primary focus on delivering high-quality patient care and support telephonically. This role is based 100% in-office, featuring a Monday-Friday schedule with alternating weekends.

Key Responsibilities:
- Act as a vital liaison for DirectRx, calling patients to ensure they receive comprehensive pharmacy services.
- Accurate and thorough input of patient data.
- Engage in comprehensive communication with patients regarding therapy.
- Schedule initial fills and refills with patients and caregivers.
- Respond promptly to patient needs.
- Educate patients on compliance as necessary.
- Support the transfer of patient prescriptions as directed by the pharmacist.

- This role operates within one of the following scheduling structures based on business needs. Are you able to meet these availability expectations? * Two weekday evening shifts (12:30 PM – 9:00 PM) and three weekday day shifts (9:00 AM – 5:30 PM), or * Monday–Friday 9:00 AM – 5:30 PM with every other Saturday or Sunday and a weekday off.
