POSITION SUMMARY:
The Integrated Decision Team Facilitator and Wraparound Certified Coach - will lead the discussion that will identify the needs and resources for the family and will establish creative and effective safety and treatment plans. At critical junctures of the case, the IDT facilitator will assess a family’s situation to determine risk level, identify options, and arrive at a shared decision on the best approach to ensure child safety, mitigate risk, and provide assistance to the family. The IDT facilitator is charged with ensuring that decisions are made via shared risk decision making. The IDT facilitator will encourage family attendance and will ensure that the staffing is in line with the program model and with high fidelity wraparound services. The IDT facilitator is the face of the agency and will need to incorporate the five promises when working with families in this setting. This position involves administrative and program development responsibilities. The IDT -facilitator is responsible for assisting in the development and monitoring of all components of the Families First Division staffing schedule from the initial staffing/wraparound family team meeting to the closure staffing. This position is also responsible for case management functions providing services to children and their families who are referred per the eligibility requirements identified by each Families First Division program and applicable contract. Case Management work involves assessment, service planning, and monitoring client progress, linking resources, coordination, high fidelity wraparound services and advocacy on behalf of the client among other responsibilities. This position requires High Fidelity Wraparound Coach Certification and BHCM certification within 6 months of employment. Responsible to meet productivity requirements of 72% which includes 1248 billable hours annually or a minimum of 104 billable hours per month.
Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential Team Members may be required to work in the field, be “on call” depending on the needs of the individual being served.
Qualifications:EDUCATION EXPERIENCE/REQUIREMENTS:
- Bachelor's Degree in Social Work or a related area of study from an accredited university
- Two years of experience working with the homeless population and/or persons struggling with severe and persistent mental illness and/or substance misuse disorders
- Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
