Hospice Chaplain (PRN)
Reports to: Director of Psychosocial Services
Job Summary: The Chaplain provides spiritual support and coordinates spiritual and/or religious services that are provided to assist patients, families, and caregivers cope with the losses that occur during the illness and eventual death of the patient. Provide spiritual and/or religious interventions to patients, families, and caregivers who need spiritual companionship and counseling as it relates to end-of-life care.
This temporary, part-time position exists to support patients in the Louisville, Fort Lupton, Loveland, and Fort Collins area, covering a leave of absence for another Chaplain. Expected availability is on Mondays from now until June 8th. Additional hours may be available PRN such as assisting with support for transitioning patients in other territories, or covering PTO for other Chaplains.
Qualifications/Educational Requirements:
- Completion of Master’s level education in theology, spiritual counseling, or other pastoral care-related studies.
- Completion of 1 units of Clinical Pastoral Education (CPE/ACPE) and/or experience with hospice spiritual counseling preferred.
- Ability to schedule and complete tasks independently.
- Ability to provide care in an interfaith and multi-cultural setting and network with local clergy and faith communities
- Excellent written and verbal communication skills
Responsibilities/Essential Functions:
The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.
- Visit the patient (primary client) and family (secondary clients) at intervals designated by the patient's plan of care. Be available for visits as requested by supervisor up to 8 hours per week.
- Document each visit or call and file in each patient’s record within 24 hours of contact.
- Complete spiritual care assessments for patients newly admitted to hospice services.
- Provide, either directly or through coordination with the patient’s/family’s religious organization, appropriate interventions for such issues as: Meaninglessness, loss of faith, failure, anger, despair, profound sadness, abandonment, betrayal, fear/dread, guilt, forgiveness, hope, and promise for the future.
- Provide a non-anxious presence, demonstrate emotional maturity, advocate for the needs of the patient
- Work as a member of an interdisciplinary team; hold professional boundaries and stay within appropriate scope of practice.
- Adhere to the ethical standards of hospice care, policies and procedures, and the Dignity Hospice Mission & Vision statements.
- Mindfully share information that is relevant to other caregivers of the patient, and reverently hold what is sensitive, personal or not appropriate to share.
Continuing Education Requirements: Employee is expected to participate in continuing education as required by Medicare or ACHC standards. This is at least 12 hours of education annually.
Scheduling & On-Call: Expected availability is 1x per week (Mondays). A PRN or part-time Chaplain can expect a case load of 5-15 patients. PRN Chaplains are not required to participate in the on-call rotation, but may voluntarily cover on-call shifts and holidays for a shift premium.
Compensation: $25 - $30 per hour DOE, plus visit mileage is reimbursed at the IRS rate.
