Pathways Hospice is a respected, Medicare Four-Star rated nonprofit serving Larimer County, western Weld County, and communities along the I-25 corridor—including Fort Collins, Loveland, Greeley, and Windsor and surrounding communities.

Our services include palliative care, expert hospice care, and grief support for individuals and families navigating some of life’s most meaningful moments.
Position Details
- Location: Fort Collins, CO
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No nights!)
- Status: Full-time (40 hours/week)
Salary: $80,080 – $90,916 annually
Plus Our Built-In Pay Advantage Program:
- Scheduled 72 hours in a 2-week pay period, get paid for 80
- Mileage reimbursement at $0.625/mile (among the highest in the industry)

Your Role: Independence with Support
As an RN Case Manager, you will:
- Lead and coordinate care throughout the hospice journey
- Build meaningful connections with patients and families
- Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders
- Provide holistic care—addressing physical, emotional, and spiritual needs
- Work independently in the field while backed by a highly supportive team

What We’re Looking For
- Graduate of an accredited school of nursing (BSN preferred)
- 1–2 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
- Current Colorado RN license and CPR certification
- Valid Colorado driver’s license, auto insurance, and ability to travel daily
- Ability to lift up to 35 lbs
