Duties:
- Provide excellent patient care in the home or facilities where the patient is located.
- Communicate with the treatment Team to ensure patient needs are met.
Establish relationships with patients and their families to meet the comprehensive needs of Hospice patients and the families caring for them.
Skills:
- Valid Registered Nurse (RN) license.
- Familiarity with electronic health record systems.
- Strong interpersonal skills to effectively communicate with patients, families, and healthcare team members.
- Ability to prioritize tasks and manage time efficiently in a fast-paced environment.
- Attention to detail to ensure accurate documentation and patient care.
