- Schedule: Full-time preferred; part-time opportunities also available

CNA Responsibilities
- Provide CNA-level care to home health and hospice patients in their homes or other places of residence, within the scope of training and certification.
- Perform covered aide services according to Medicare home health and hospice guidelines.
- Assist patients with activities of daily living and other personal care needs based on assessments, individualized care plans, and direction from nursing management.
- Provide care under the supervision and direction of a Registered Nurse.
- Develop positive and professional relationships with patients, families, physicians, interdisciplinary team members, and other healthcare professionals.
- Monitor patients and promptly report changes or concerns to the appropriate member of the care team.
- Accurately document observations, services provided, and patient care in a timely manner.
- Safely and appropriately use patient-care equipment within the CNA's training and competency, including wheelchairs, hospital beds, side rails, oxygen equipment, and other applicable medical equipment.
- Follow established care plans, safety procedures, and organizational standards while providing respectful and compassionate care.
Required Licenses & Certifications
- Active Utah CNA certification
- Current CPR certification
- Valid Utah driver’s license
- Current automobile insurance
