Responsibilities

• Medication management & chart auditing
• Performing resident treatments as ordered per the care plan
• Observing, tending to and documenting residents health needs
• Communicating with RN regarding relevant staff and resident information and/or concerns
• Reviewing appointments and orders, follow up communicating follow up appointments with the front desk
• Processing MD orders completely and accurately
• Completing incident reports as needed
• Ensure the staff is performing safe practices
• Reviewing messages and communicating with staff through Residex
• Performing wound care, communication with Home Care, managing supplies
• Assist with compliance audits and staff guidance
• Focused assessments for clinical needs
  
  

Qualifications

• Possess a valid Minnesota LPN License
• Minimum of 1 year experience

• Leadership ability
• Must be able to occasionally fill in when needed during staffing shortages
• Maintaining confidentiality and professionalism at all times
