Responsibilities
- Medication management & chart auditing
- Performing resident treatments as ordered per the care plan
- Observing, tending to and documenting residents health needs
- Communicating with RN regarding relevant staff and resident information and/or concerns
- Reviewing appointments and orders, follow up communicating follow up appointments with the front desk
- Processing MD orders completely and accurately
- Completing incident reports as needed
- Ensure the staff is performing safe practices
- Reviewing messages and communicating with staff through Residex
- Performing wound care, communication with Home Care, managing supplies
- Assist with compliance audits and staff guidance
- Focused assessments for clinical needs
Qualifications
- Possess a valid Minnesota LPN License
- Minimum of 1 year experience

- Must be able to occasionally fill in when needed during staffing shortages Maintaining confidentiality and professionalism at all times
