PURPOSE AND SCOPE:

Supports the mission, core values, and customer service of Dialysis Care Center, LLC (DCC).

Adheres to the DCC Policies and Procedures and Compliance Program, including all state and federal 

regulatory requirements.

The In Center Hemodialysis Registered Nurse provides safe, high-quality patient care while 

coordinating daily clinical operations, supervising direct patient care staff, ensuring regulatory 

compliance, promoting operational excellence, and supporting Dialysis Care Center's mission of 

exceptional patient outcomes..

PRIMARY DUTIES / ACTIVITIES:

• Supervises and mentors the Patient Care Technicians (PCTs) and directs the day-to-day patient care.

• Reviews new staff’s documentation for accuracy.

• Utilizes DCC Labor Logic to optimize staffing, labor productivity, scheduling efficiency, overtime 

management, and patient treatment scheduling while maintaining safe staffing standards.

• Works with Clinical Nurse Manager (CNM) on developing optimal employee staffing and patient 

schedules, and Coordinates workflow during assigned shift.

• Creates, maintains, and communicates efficient and timely employee schedules according to the

needs of the facility.

• Completes assigned Daily Opening and Closing Procedures

• Monitors DCC Quality Metrics: Kt/V, Anemia. Catheter Rate, Access Infections, Missed treatments, 

etc.

• Participates in DCC Missed Treatment Intervention initiatives by assisting with patient outreach, 

rescheduling, and interdisciplinary communication regarding missed or shortened treatments.

• Serves as a resource/subject matter expert for patient needs and issues, staff education, and in service sessions as necessary, working closely with the educators, outcome managers, Anemia 

managers, and DCC Dashboard specialist.

• Monitors and documents on the DCC Dashboards.

• Supervises and performs direct patient care, including but not limited to taking and monitoring vital

signs, pre-treatment assessment, initiation, monitoring, termination, access hemostasis, and post treatment assessment of dialysis

• Responsible for documentation, physical nursing assessment of patients, administering dialysis 

medications, transcribing physician orders, assessing dialysis adequacy, drive catheter reduction.

• Assist with patient scheduling, conduct daily clinical huddles, delegate patient assignments, and assist

with staff coaching.

• Reviews documentation for completeness and regulatory compliance.

• Reviews laboratory orders, monitors laboratory results through Ascend, ensures proper specimen 

collection, verifies correct tubes are used, ensures proper packaging and shipment of specimens, and 

promptly communicates critical laboratory values to the physician.

• Communicates all emergencies, patient life-threatening complications, and related situations to the 

attention of the medical director, physician, and Clinic Manager. Maintains competency with all 

emergency operational procedures. Ensures verification and availability of adequate emergency 

equipment.

• Assesses patients’ response to hemodialysis treatment therapy, making appropriate adjustments 

and modifications to the treatment plan as prescribed by physician, assessing target weights.

• Ensures compliance with safety and infection preventative measures, performing safety and 

infection audits on a routine basis.

• Ensuring patient safety and monitoring adverse Event Occurrences.

• Educates patients and Family members on all aspects of End Stage Renal Disease.

• Collaborates with other disciplines to ensure patient outcomes are being met, and instructs patients 

on pre and post dialysis care.

• Provides ongoing continuous education to patients regarding their renal disease, vascular access 

and dialysis therapy, fluid management, dietary modifications, medication compliance, discuss 

modality awareness and transplant awareness.

• Successfully completes all DCC University training modules.

• Maintains annual competency validation.

• Train new and current staff as needed. Assists with the orientation of new employees. Assists with 

competency validation.

• Participates in mandatory continuing education

• Complete Staff Nurse Water Quality Facility Training

• Participates in all required staff meetings.

• Participation in care conferences and in QAPI

• Float between facilities as needed and work a flexible schedule (including mornings, evenings, 

weekends, and holidays)

• Staff scheduled hours are based on the needs of the facility and are subject to change.

• Ensures mandatory in-services and annual competencies are completed as required.

• Participates in all required staff meetings.

• Travel to other locations as needed..

• Other duties, as assigned.

CUSTOMER SERVICE:

• Responsible for supporting the values and customer service standards of HDS/DCC.

• Accountable for outstanding customer service to all staff, clients, and vendors

• Develops and maintains effective relationships through effective and timely communication.

• Initiates action to respond, resolve, and follow up regarding customer service issues with

all customers promptly.

EDUCATION/ QUALIFICATIONS:

• Graduate of an accredited School of Nursing, ADN required; BSN preferred

• Current state licensure as a Registered Nurse

• Maintains state licensure in good standing.

• Current CPR Certification

EXPERIENCE:

• Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft 

Office Suite: Outlook, Word and Excel a must • Good presentation & communication skills

• Must be productive & resourceful with the ability to meet deadlines

OTHER QUALIFICATIONS:

• A minimum of 6 months hemodialysis experience preferred.

• Day-to-day work includes desk and personal computer work and interaction with patients, facility staff, 

and physicians. Working hours can vary, including the days, nights, weekends, and holidays. Travel 

to scheduled staff meetings may be required. The position requires travel between assigned facilities 

and various locations within the community. Travel to regional, Business Unit, and Corporate 

meetings may be required.

• The work environment is characteristic of a health care facility with air temperature control and 

moderate noise levels. May be exposed to infectious and contagious diseases/materials

The position provides direct patient care that regularly involves heavy lifting and moving of patients and 

assisting with ambulation. Equipment aids and/or coworkers may assist. This position requires frequent, 

prolonged periods of standing and the employee must be able to bend over. The employee may 

occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee 

may be required to frequently lift and lower solutions of up to 30 lbs. and occasionally lift solutions of up to 

50 lbs., as high as 5 feet.
