What are the responsibilities and job description for the Medical Coordinator/Case Aide position at Diakon?

General Responsibilities:
- Ensures effective operations through selection, training, motivation, evaluation and development of a competent on-site medical coordination team.
- Formulates treatment authorization request to Point Comfort Underwriters for unaccompanied refugee clients, as needed.
- Serves as the subject matter expert of Point Comfort Underwriters Insurance.
- Schedules and facilitates initial medical screenings, follow up visits, and any other medical/dental related appointments as required by ORR policy.
- Develops, recruits, and maintains positive working relationships between contracted providers to include all health providers and related partners.
- Manages and maintains the record-keeping process to ensure there is an effective filing system in which information is forwarded smoothly.
- Ensures documentation and filing compliance with all required forms in accordance with the agency, state licensing, and ORR.
- Effectively trains staff members on documentation compliance in accordance with agency, state licensing expectations, and ORR, as needed.
- Reconciles and addresses all billing errors to ensure accurate billing with no profit loss.
- Maintains accurate, up-to-date information on foster children in care while meeting DHHS, ORR, and internal documentation and reporting requirements.
- Stays abreast of all agency, federal, and state regulatory requirements related to social services.
- Periodically travels to health care provider facilities to build positive working relationships and transport clients.
- Provides supervision of youth in care.
- Creates client intake case files with necessary intake documents and ensure that youth are properly enrolled in UC Portal platform.
- Assists with compiling electronic and physical files for ORR or State Licensing representatives, upon request.
- Assists with filing, closing, and with overall daily maintenance of records found in either the physical case file and on UAC Portal.
- Facilitates incoming calls to youth with appropriate family members and other approved caregivers and ensure the provision of weekly telephonic contacts with family of origin, primary caregiver, and/or sponsor.
- Assists with the coordination of fingerprint services for family reunification at program designated digital site.
- Attends internal and external training and apply newly gained knowledge in providing services.
- Attends and prepares for required court hearings (if applicable).
Education/Training: Minimum of one (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care AND any of the following: Licensed practical or vocational nurse (LPN/LVN) or higher health-related qualification (e.g., licensed registered nurse (RN) with a bachelor’s degree), OR bachelor's degree in a relevant field.
Experience: Experience: At least one (1) year of experience working with medical providers and at least one (1) year employment experience in the child welfare field working with children and/or adolescents. 30 plus hours of higher education in a social service field, preferred. Bilingual Spanish/English fluency, preferred.
Skill(s): Bilingual in English and Spanish, preferred; Prior work experience with refugee and/or immigrant populations preferred; mature and stable judgment as well as sensitivity to various cultures and the unique history of refugees; ability to work under stress or in fast-paced environment; ability to work independently and exercise a high level of confidentiality; computer and typing skills sufficient to perform essential job functions; valid driver’s license, proof of automobile insurance and positive driving record to transport minors in own vehicle if required; excellent verbal and written communication skills; must have ability to provide excellent and compassionate customer service; must be reliable with time sensitive deadlines and tasks; work well under pressure and adaptable to change.
