Position Overview:
Diadem Hearts Inc. is seeking a compassionate and motivated Licensed Chemical Dependency Counselor to provide quality care and support to individuals experiencing substance use disorders. This role involves assessments, treatment planning, counseling, and coordination of care to promote recovery and long-term stability.
Requirements:
- Must be licensed by the Texas Department of State Health Services or eligible for Counselor Trainee status
- Bachelor's degree in psychology, Social Work, or related field (preferred)
- Relevant experience may substitute for education
- If in recovery, must have at least two (2) years of continuous sobriety
- Must have reliable transportation
Key Responsibilities:
- Conduct client screenings, intake assessments, anddetermineeligibility for services
- Develop individualized treatment plans with measurable goals
- Provide individual, group, and family counseling sessions
- Educate clients and families on substance use, recovery, and available resources
- Maintainaccurateandtimelydocumentation (assessments, progress notes, discharge summaries, etc.)
- Coordinate care with internal teams, community providers, and support systems
- Assistclients with referrals to community resources when needed
- Motivate clients to engage in treatment and continuing care programs
- Maintain client confidentiality and uphold ethical standards
- Participate in team meetings and clinical staffing
Skills & Abilities:
- Strong knowledge of substanceusedisorders and treatment modalities
- Understanding of co-occurring mental health conditions
- Ability to manage crisis situations calmly and professionally
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and documentation skills
