Role Description
This is an on-the-road role for a Hospice Community Liaison. The position is all about building authentic relationships, engaging with community partners, and helping people understand who we are and how we support families.
You won’t be doing this alone—we have great tools for outreach, including:
- A music therapist who can provide interactive sessions
- Team members who can assist with life enrichment and community engagement activities
- Owners who are hands-on and available
- A clinical team ready to provide education, presentations, and support
You’ll have the freedom to be creative and find meaningful ways to connect with the community. No micromanagement—just clear expectations and support.
Key Responsibilities:
- Build and maintain relationships with healthcare providers, senior living communities, and community organizations
- Educate partners about our services and what makes Devotion Hospice different
- Identify individuals who may benefit from hospice care and help facilitate admissions
- Support smooth transitions for patients and families
- Participate in community events, outreach activities, and engagement opportunities
- Use creativity and genuine connection to represent our mission

Qualifications
- Experience in hospice, palliative care, or end-of-life care
- Experience in healthcare outreach, sales, or liaison work
- Strong communication and interpersonal skills
- Ability to work independently without constant oversight
- Previous experience as a hospice liaison or similar role required
