As a Hospice Aide you will: Key Responsibilities:Provide assistance with at least 2 of the following ADL's including personal hygieneAssist patients with medication intake and prescribed exercisesIncidental household services, which are essential to patient health careWill follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visitsBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care Skills and Experience Required: Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for OhioMust maintain a valid driver's license, good driving record and reliable automobileHigh school graduates or GED preferredDemonstrate the ability to be able to work within a multi-disciplinary team settingDemonstrate the ability to work in field situations**THIS IS A PRN POSITION - FILLING IN AS NEEDED FOR EITHER ANN ARBOR OR LANSING AREA - DEPENDANT UPON WHERE AIDE RESIDES.**
