- Oversee and monitor the implementation of ISPs to ensure that goals, objectives, and supports are effectively carried out.
- Conduct monthly and quarterly home visits to assess service quality, environment safety, and individual satisfaction.
- Review, approve, and provide feedback on all documentation, including daily notes, monthly summaries, and progress reports in Therap.
- Ensure documentation meets DBHDD, Medicaid, and CSS Healthcare standards of accuracy and timeliness.
- Support host home providers and DSPs in understanding ISP objectives, health risk categories, and individual behavior support needs.
- Participate in Interdisciplinary Team (IDT) meetings, ISP reviews, and service coordination discussions.
- Ensure Health Risk Screening Tools (HRSTs) are completed, updated, and consistent with the individual’s medical and behavioral needs.
- Monitor incident reports to ensure timely completion, submission, and corrective action follow-up per DBHDD Policy 02-807 and agency procedures.
- Provide training, technical assistance, and mentoring to DSPs and host home providers on person-centered practices, documentation, and behavioral supports.
- Assist in developing Behavior Support Plans, Risk Management Plans, and Safety Plans as needed.
- Maintain professional communication with families, guardians, and external professionals to coordinate care and advocate for individual rights and choices.
- Collaborate with the Program Director and Compliance Team to ensure service delivery remains in alignment with DBHDD Quality Standards and agency performance benchmarks.
- Monitor Personal Needs Allowance (PNA) records, personal funds, and other individual financial documentation for compliance and accountability.

Pay: $35,000.00 - $40,000.00 per year
