What are the responsibilities and job description for the Support Coordinator (RN)-Preceptor position at Detroit Area Agency on Aging?
Develops and completes participants care plan; establishes goals and directives of the care plans, and services to be utilized. Maintains regular, ongoing contact with clients, caregivers, and physicians. Problem solving skills by creating strategies to assist participants enhance their environment is a needed attribute. This position is expected to possess the ability to assimilate their professional knowledge, and skills to create a person-centered plan of care while engaging participants with community resources when applicable. Completes and submits timely paperwork and reports.
