- Communicate with patients, staff and vendors to coordinate and arrange transport services based on health plan benefits.
- Coordinate transportation services for patients without transport benefits when approved by management.
- Review transport claims, input transport requests, and create/input authorizations into electronic database.
- Review errors received from claims department and correct information in database.
- Assist with Post Utilization follow-up care as a back-up to primary support coordinators.
- Process referrals and enter data into q.Auth to create prior authorization to specialty care providers post utilization when functioning as back-up.
- Monitor DOHC Patient Helpline and answer incoming calls in a timely manner to address, manage, and/or direct questions or concerns to the appropriate department/staff.
- Monitor DOHC Transportation phone line and answer incoming calls in a timely manner to address, manage, and/or direct questions or concerns regarding routine transportation.
- Training of new team members in transportation requests and authorizations for facility transfers, routine appointments, etc.
- Refer patients to the DOHC Immediate Care Clinics and the Patient Helpline for any questions, concerns, or for assistance.
- Management ofreferrals in process in Traxit referral tracking software.
- Perform other duties as assigned.
- Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience
- Be an active listener (make eye contact, validate)
- Be sensitive to factors that influence customers and co-worker’s situation including age, gender, culture, race and socioeconomic status. Be observant of others’ social cues (emotions) and respond appropriately
- High school diploma or equivalent.
- 2 years of experience in the medical field. Experience in Home Health setting or discharge planning, preferred.
- Knowledge of medical terminology.
- Ability to proficiently operate a computer.
- Ability to interact effectively with all levels of the organization.
- Bi-lingual preferred.
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
- Sitting: Approximately 75% of day

- Lifting: 0 - 40 lbs (equipment, supplies) - approximately 5% of day
- Bending: Approximately 10% of day. Kneeling < 20%.
- Hearing/Visual Acuity: Adequate for use with computers, telephone and or Blackberry -
- Computer: Highly technical work environment - Must be able to work ≥ 6 hours / day using keyboard, mouse and monitor
- Reaching: Above head 75 degrees - approximately 5%.
- Hand grip dexterity: Approximately 40% of day
