What are the responsibilities and job description for the RN, Triage (Night Shift 8:00 pm - 8:00am) position at Desert Oasis Healthcare?
Provides telephonic triage and direct patient care after normal business hours. Follows care plan, provides treatments, and works collaboratively with the members of the team to help meet positive patient care outcomes. Directs and supervises LVN's as necessary.

- Provides direct patient care and telephonic triage, as defined in State Nurse Practice Act.
- Responds to urgent and family/patients calls during "after normal business hours" through collaboration with on-call contracted answering service.
- Directs and supervises LVN's to respond to on-call needs and patient visits.
- Provides accurate and timely documentation of patient services to reflect the plan of care, including submission to the of office.
- Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
- Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (CHHA, OT, PT, MSW, Dietitian, Manager, ST).
- Ensures medications profiled with HP in a timely manner and DME ordered as appropriate.
- Assist with the continuity of care/changes in LOC (GIP/Respite/Routine SNF admissions) and discharge planning from facilities as required.

- Current California RN license required.
- Minimum of one year clinical nursing experience required. Previous Home health or hospice experience, preferred.
- Understanding of guidelines and scope of nursing practice.
- Able to effectively communicate with all team members and customers.
- Able to work independently with minimal supervision to ensure that patient care is adequate and appropriate.
- Able to work as part of interdisciplinary team to effectively manage patient care.
- BLS required within 60 days of hire.
- Current drivers license and proof of insurance required.
- Sitting: Approximately 50% of day
- Standing: Approximately 35% of day
- Walking: Approximately 15% of day
- Lifting: 0 - 50 lbs - approximately 10% of day
- Bending: Approximately 10% of day. Kneeling < 10%.
- Hearing/Visual Acuity: Adequate for use with computers, telephone and or Blackberry -
- Computer: Highly technical work environment - Must be able to work ≥ 6 hours / day using keyboard, mouse and monitor
- Reaching: Above head 75 degrees - approximately 25%.
- Hand grip dexterity: Approximately 70% of day
