The Clinical Pharmacist in the Population Health and Prescription Management (PHARxM) provides clinical, and educational functions as necessary to support the overall patient care provided by PHARXMTM Services. The Clinical Practice Pharmacist provides medication therapy management for patients as defined under medical staff approved protocols. This individual supports and contributes to process improvement within the PHARxM clinics and department and participate in the training end mentorship of Pharmacy Students and Pharmacy Residents.

##### Professional Duties
- Follow medical staff approved protocols in management of patients defined under PHARxM department.
- Provide comprehensive medication therapy consults to patients referred to PHARxM.
- Collaborate with the interdisciplinary team to ensure optimal patient care coordination.
- Document patient interactions and consultation's in the patient's electronic medical/health record (EMR/EHR) in accordance with established departmental policies, procedures and timeframes.
- Conduct critical patient monitoring and comprehensive chart reviews to identify, prevent, and mitigate drug-related problems, including adverse drug reactions, drug interactions, medication omissions, and inappropriate use.
- Conduct thorough medication histories, assess patient compliance/progress, and suggest modifications to achieve desired treatment outcomes.
- Provide a consistent process of patient care that ensures the appropriateness, effectiveness and safety of the patient's medication use.
- Utilize electronic health records (EHR) to document interventions and track patient outcomes.
- Evaluate complex medicine regimens, identify potential interactions, and recommend evidence-based therapeutic alternatives to optimize efficacy and safety.
- Make appropriate evidence-based, patient-centered medication recommendations.
- Provide education to patients to promote health and wellness.
- Maintain proficiency as a preceptor and actively demonstrate education and mentorship skills by developing student, resident, and staff training experiences/competencies and creating new training opportunities.
- Participate in on-call schedule.
- Perform other duties as assigned.

- PharmD, BS in Pharmacy, or foreign equivalent, with a valid CA RPh license required.
- Current California Pharmacist License required. Board Certification, preferred.
- 2-5 years of ambulatory care experience required. PGY1 preferred. PGY1 training may substitute 3 years of ambulatory care experience.
- Active member of a State or National Pharmacy Organization, preferred.
- Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
- Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
- Ability to work independently and as part of a team.
- Ability to manage time effectively and prioritize tasks to meet established deadlines.
- BLS required within 60 day of hire. BLS not required for employees who are not involved in direct patient care.
