What are the responsibilities and job description for the Patient Field Service Liaison position at Desert Oasis Healthcare?
The Patient Field Services Liaison is responsible for monitoring Part C quality measures and conducting patient outreach and education to support the achievement of 5 star benchmark rating. This individual identifies opportunities to close gaps in quality measures and collaborates with other staff & departments to facilitate overdue screenings/services to patients. This individual supports continuity of patient care by providing in home patient visits to perform blood draws, assist patients with virtual visits and facilitate communication with patients.

- Monitor Part C quality measure reports and contribute towards the achievement of 5 star benchmark on Part C quality measures.
- Participate in health fairs to support closing gaps in quality measures.
- Identify opportunities for patient outreach and collaborate with department management to establish priority.
- Conduct patient outreach and education to support closing quality measure gaps.
- Coordinate virtual visits and diagnostic testing to close gaps in quality measures.
- Conduct in home visits to assist patients with completing virtual visits with other departments within the organization.
- Conduct in home visits to facilitate connection between patient and other departments within the organization after 3rd failed contact attempt.
- Collaborate with quality partners to support patient outreach.
- Perform in home blood draws for patients who don't qualify for in home lab services upon receipt of laboratory order.
- Document patient interactions in the patient's electronic medical/health record (EMR/EHR) in accordance with departmental policies, procedures and deadlines.
- Perform other duties as assigned.

- High School diploma or equivalent.
- Current California Phlebotomist license.
- 2 years of phlebotomist experience in a laboratory, healthcare or managed care setting.
- Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
- Ability to utilize electronic medical/health record system/software.
- Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
- BLS required within 60 days of hire.
- Current California driver’s license and proof of auto insurance.
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
