Position Summary
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our Nephrology team. The NP will provide comprehensive care to patients with acute and chronic kidney disease, including hypertension, electrolyte disorders, and end-stage renal disease. This role involves close collaboration with nephrologists, nurses, and interdisciplinary care teams to deliver high-quality, patient-centered care.
Key Responsibilities
- Evaluate, diagnose, and manage patients with renal and related conditions
- Conduct patient assessments, histories, and physical examinations
- Order, interpret, and manage laboratory and diagnostic studies
- Develop and implement individualized treatment and care plans
- Manage dialysis patients (hemodialysis and/or peritoneal dialysis, if applicable)
- Provide patient and family education on kidney disease management and prevention
- Collaborate with physicians and healthcare team members to optimize outcomes
- Maintain accurate, timely clinical documentation in the EMR
- Ensure compliance with clinical guidelines, safety standards, and regulations
Qualifications
- Master’s or Doctoral degree from an accredited Nurse Practitioner program
- Current Nurse Practitioner license in [State]
- National certification (ANCC or AANP)
- Active DEA license (or eligibility to obtain)
- Prior experience in nephrology or internal medicine preferred (new graduates with strong interest may be considered)
- Strong clinical judgment, communication, and organizational skills
Preferred Experience
- Experience managing CKD, ESRD, hypertension, and dialysis patients
- Familiarity with outpatient nephrology clinics and/or hospital consult services
- Experience with electronic medical records
