What You'll Do
- Provide skilled nursing care to pediatric patients in their homes
- Develop, implement, and oversee individualized plans of care
- Perform comprehensive patient assessments and ongoing evaluations
- Coordinate care with physicians, therapists, caregivers, and other members of the healthcare team
- Educate patients and family members on disease management, medications, treatments, and home care
- Monitor patient progress and communicate changes in condition promptly
- Complete accurate and timely clinical documentation in accordance with state, federal, and agency guidelines
- Ensure the highest standards of patient safety, quality care, and family-centered service
Qualifications
- Current Arizona Registered Nurse (RN) license in good standing
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment, communication, and critical thinking skills
- Excellent organizational and time-management abilities
- Passion for providing compassionate, family-centered pediatric care
- Pediatric, home health, or case management experience preferred, but not required
- Current BLS certification
