Key Responsibilities
Provide skilled nursing care in accordance with individualized care plans
Monitor patient conditions, administer medications, and report changes to the healthcare team
Educate patients and family members on disease management and care needs
Complete accurate and timely documentation in compliance with home health regulations
Maintain high standards of patient safety and quality care
Qualifications
Active Arizona Registered Nurse (RN) license in good standing
Valid driver’s license and reliable transportation
Strong clinical, communication, and organizational skills
Commitment to compassionate, patient-centered care
Home health experience preferred but not required
