Your Role as a Physical Therapist:
Assess and evaluate patients' therapeutic, rehabilitative, and functional status to develop a personalized care plan.
Evaluate home environments to ensure optimal recovery conditions.
Assess muscle strength, mobility, gait, and range of motion (ROM).
Implement and oversee physical therapy treatment plans based on physician recommendations.
Educate and guide patients, caregivers, and family members on therapy techniques and goals.
Identify additional home health services needed and make appropriate referrals.
Document and track patient progress through clinical reports.
Provide in-service education for nursing staff as needed.
Participate in peer consultations for continuous professional growth.
Supervise Physical Therapy Assistants per company policy and state regulations.
Qualifications:
Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
Active state licensure as a Physical Therapist.
Minimum of two years of experience as a Physical Therapist (home health experience preferred).
Strong verbal and written communication skills and organizational abilities.
Current CPR certification.
