Your Role

As a Home Health Physical Therapist, you’ll provide individualized therapy to patients recovering from injuries, surgeries, illnesses, or other medical conditions. You’ll play an important role in helping patients improve mobility, reduce pain, increase independence, and achieve their personal goals.

Key Responsibilities

• Evaluate patients in their homes and develop individualized plans of care

• Provide therapeutic exercises, mobility training, manual therapy, and other appropriate interventions

• Help patients improve strength, balance, coordination, mobility, and overall function

• Educate patients and families on exercises, safety, fall prevention, and continued progress

• Monitor patient outcomes and adjust treatment plans based on individual needs

• Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary care team

• Maintain timely and accurate clinical documentation

• Support patients in achieving greater independence and quality of life at home

• Stay current with professional standards and physical therapy best practices

What We’re Looking For

• Current Physical Therapist (PT) license in Arizona

• Strong clinical assessment and problem-solving skills

• Excellent communication and interpersonal skills

• Ability to establish trust and rapport with patients and families

• Ability to work independently in the field while collaborating with an interdisciplinary team

• Strong organizational and time-management skills

• Commitment to providing compassionate, patient-centered care
