What are the responsibilities and job description for the Support Coordinator position at Dent County Developmental Disabilities Board?
Dent County Developmental Disabilities Board is hiring for a Support coordinator. The position involves working with an assigned group of individuals with developmental disabilities and their families, as well as agencies which provide services. The essential functions of the job include developing support plans, advocating for and assisting persons to obtain services and supports they need, coordinating the delivery of those services, monitoring implementation of support plans, maintaining accurate records in compliance with state and federal requirements, and assist in the development of new services/supports as may be needed.
Applicants must have a bachelor’s degree from an accredited four-year college or university.
The work involves travel to persons’ homes and agency sites, so applicants must have a valid Drivers License and reliable means of transportation available daily. Applicants must also have skills using a personal computer and related software for completing written documentation, as well as skills using of other typical office equipment. Must have knowledge of community resources. Applicants must be able to complete work related tasks within specific deadlines. Salary will be based on experience.
