One of our clients is seeking Intensive Care Coordinators in the Chicago south side area.CLIENT'S AVAILABLE HOURS* 8am-5pm Mon-Fri, with some flexibility to start early or work later as needed.SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY* The Coordination and Support: Intensive Care Coordinator (CCSI) takes primary responsibility for makingthe care coordination process happen for children with a mental health diagnosis and their families throughthe facilitation of Child and Family Team Meetings, coordinating with professionals, and helping the childmeet their goals. CCSI is provided to children stratified into Tier 2. Designated CCSI Care Coordinatorswork with an average of 16 families (based on population) at a time and are never assigned to work withmore than 18 families at once.
