What You’ll Do
As a Home Health Physical Therapist , you’ll deliver high-quality physical therapy services while working alongside physicians, nurses, and fellow rehab professionals to create meaningful outcomes for every patient.
Your day-to-day will include:
- Evaluating patients and developing individualized treatment plans
- Delivering hands-on therapy, therapeutic exercise, and functional training
- Educating and motivating patients to reach their mobility and recovery goals
- Utilizing a wide range of treatment modalities (manual therapy, hydrotherapy, ultrasound, traction, and more)
- Monitoring progress and adapting treatment plans for optimal results
- Collaborating on discharge planning and designing home exercise programs
- Supervising Rehab Aides and supporting staff development
- Maintaining accurate documentation and ensuring patient confidentiality
- Promoting a safe, clean, and compliant treatment environment
What We’re Looking For
- Active Colorado Physical Therapy License (required)
- BLS certification (required)
- Previous experience preferred—but motivated new grads are welcome to apply
- Strong clinical skills and a patient-centered mindset
- Excellent communication and documentation abilities
- Commitment to quality care, teamwork, and professional growth
Physical Requirements
- Ability to stand, walk, bend, stoop, squat, and assist patients throughout the day
- Medium physical demand level
- Lift up to 50 lbs occasionally and 20 lbs frequently
- Full range of motion and manual dexterity to operate equipment and technology
