Delta Community Supports, Inc. (“Delta”) is seeking a passionate and reliable team member to serve as a Licensed Practical Nurse (LPN) on our Nursing team at Drexel Hill, PA area . The Nurse reports to the Registered Nurse and the Director of the Community Center.

The LPN will promote the community support of people with developmental and intellectual disabilities. Responsible for a variety of practical nursing and care duties for individuals we support, including preparing, administering and recording medication and treatments, and providing first aid at the Community Center.

Duties and Responsibilities:

  • Provide first aid to individuals and staff.

  • Administer and record medications in medication log.

  • Request refills of medication as needed from providers or family members.

  • Monitor vital signs as indicated by physicians’ order or medical need.

  • Assess need for medical treatment; notify appropriate contact person (family or provider) when an individual is injured or ill and may be in need of medical treatment.

  • Observe individuals for behavior change, response to medication, side effects, objective and subjective signs of physical illness and refer for treatment if necessary.

  • Communicate with management staff regarding medical issues of individuals.

  • Track annual physicals for individuals served at the Community Center:

  • Send reminders to families/residential providers to update physical

  • Review physicals for completeness and significant changes in medical conditions.

  • Update special precaution lists for individuals with seizure disorders, dietary restrictions, allergies and special needs during as needed or on an annual basis.

  • Perform quarterly nursing review for individuals.

  • Provide quarterly infectious disease training to staff.

  • Train staff in healthcare issues such as seizures, hepatitis, scabies, diabetes, dysphagia, recognizing signs of illness and other relevant medical/health concerns.

  • Assist with direct care service such as feeding, personal care and toileting.

  • Participate as part of the interdisciplinary team as requested, on health care issues.

  • Act as a resource to staff in health care issues.

  • Attend meetings and write reports as required/requested.

  • Review all incident reports that pertain to illness and/or injury and document follow-up.

  • Complete Lifetime Medical History on all newly admitted individuals, update as needed and review annually thereafter.

  • Ensure that all first aid boxes are complete and fully stocked; and with current dates when applicable.

  • Order supplies for first aid boxes and nursing station as needed.

  • Report any unsafe conditions immediately to supervisor.

  • Perform such other duties as assigned.

  • This position also requires the LPN to drive individuals to their medical and other related appointments as part of their regular duties.

Qualifications:

 

  • ASN required; BSN preferred

  • Graduate from an accredited school of nursing program

  • Current, unrestricted license to practice as a Licensed Practical Nurse in the state of assignment.

  • Two (2) years of experience providing community based supports (residential)

  • Must hold a valid state LPN license.
