What are the responsibilities and job description for the Support Coordinator position at DeLA Case Management?
The Support Coordinator will conduct initial CPOC meetings, Annual CPOC Meetings,, required Revision meetings and quarterly review meetings in the OCDD Population (NOW, Children's Choice, ROW and Supports). The Support Coordinator is also responsible for completing monthly phone contacts with families and providing resource advocacy to connect the family to local and state resources. Prepare comprehensive plans of care for children and adults in the Region 6 area (Avoyelles, Catahoula, Concordia, Grant, LaSalle, Rapides, Vernon and Winn). Reimbursed travel required throughout Region VI. At least a Bachelor's degree in Human Services Field required. Previous experience as Support Coordinator preferred. This is a supervisory track position.
