Job Details

Under the direct supervision of the physician or RN, the Licensed Practical Nurse (LPN) provides basic ambulatory nursing care of pediatric patients aged one day to 21 years. Nursing care includes performing obtaining vital signs and specimens, recording basic history and complaints, reviewing medications and sleep therapies. Office duties include scheduling patients for sleep clinic and sleep studies, assisting with medication prescription refill processing and prior-authorizations, relaying patient lab results and treatment plans as directed and patient follow-up for treatment compliance and reinforcement of care plan. The LPN will participate in health care decision making by communicating observations to the health care team. Documentation of observations will follow the department guidelines. Reports to the Shift Coordinator/ Administrative Manager and interacts interdepartmentally with other divisions.

Hours: Days will vary, M-F 16 hours per week

  • Clinical Care
  • Maintains knowledge of current tests and treatment for the diagnosis and management of sleep disorders
  • Provides basis ambulatory nursing to patients in the sleep clinic, including obtaining vital signs, performing pulse oximetry, obtaining specimens, recording basic history and concerns/complaints, reviewing and updating medication list and reviewing sleep therapies (CPAP, etc.).
  • Provides nursing care with regard to the patient age/developmental level, comfort level and wellbeing.
  • Understands what to look for in the results of various lab tests commonly ordered for sleep related disorders and communicates these results to the provider and patient as directed.
  • Gathers information regarding patient/family compliance with prescribed sleep plan of care, when applicable.
  • Knows when and how to communicate to physicians/providers and sleep team to report new or continued concerns, symptoms or treatment issues
  • Maintains competence in the application of sleep testing equipment (Actigraphy), current options for CPAP devices and interfaces, and the downloading of patient data from such devices.
  • Promotes family centered care, acting as the family liaison to the providers and care team to report any concerns, dissatisfaction or personal family beliefs that interfere with compliance with prescribed care. Works to alleviate those concerns.
  • Acts as a child health advocate relaying any child welfare concerns to the provider.
  • Contacts social work or other internal resources when families need financial or social support
  • Collaborates with DME companies for CPAP equipment and interfaces.
  • Documents all observations, complaints, concerns, treatment compliance, etc. in Epic.
  • Provides patient/family with After Visit Summary of their clinic visit and reviews content.
  • Schedules follow-up appointments in Epic for sleep clinic or other internal or external specialists as directed by the provider. Completes paperwork as required.
  • Participates in weekly sleep huddles as requested.
  • Provides timely follow-up for patient/family phone requests for prescription refills, patient calls need for rescheduling, etc.
  • Pursues pre-certification/pre-authorization for sleep testing and medications as required.
  • Manages prescription refill requests, communicating with the prescribing provider, family specified pharmacy and the family.
  • Collects OARS report data as requested.
  • Patient /Family Education
  • Explains medical tests, procedures, medications and treatment/care plan to patient and family using terminology that is appropriate for their understanding, learning preference and education/developmental level.
  • Reports any perceived limitations or barriers to learning to provider.
  • Seek interpretation services as needed for those families who do not speak English.
  • Utilizes sleep clinic educational materials to augment dialogue.
  • Reinforces provider plan of care.
  • Documents all patient education in Epic.
  • Patient Safety
  • Demonstrates an awareness for a safe patient care environment, proactively identifying and mitigating potential hazards for patients, visitors, and employees while carrying out daily work responsibilities.
  • Encourages patients/families to ask questions about their medications, procedures and tests to improve the safety of patient care delivery.
  • Appropriately reports any safety concerns or incidents through the WBIR or to the sleep coordinator when appropriate.
  • Limits the use of verbal orders according to hospital policy
  • Assists in checking for expired supplies in work areas.
  • General Staff Expectations:
  • Demonstrates DCH Values in daily work.
  • Acts as a positive role model in the sleep clinic, office and lab, mentoring others in sleep-related observations and care.
  • Assists with orientation of new employees as requested.
  • Dresses professionally per hospital policy.
  • Reads email on a timely basis to stay abreast of hospital and department updates.
  • Attends monthly sleep staff meetings and hospital GPS sessions.
  • Completes mandatory education during birth month.
  • Documents all continuing education in HLC or on IPR annually.
  • Maintains current Ohio license.
