DayBreak Senior Services is Northern Utah’s only licensed Adult Day Health Facility, specializing in dementia-informed care and respite services. We’re proud to provide structured, meaningful days for up to 24 older adults while giving families peace of mind and much-needed support.
We are seeking a licensed professional to lead and expand our therapeutic programming and engagement services. This role will serve as a subject matter expert, consulting closely with the Program Director while supporting our entire care team. We welcome Licensed Professionals in related fields with proven experience serving older adults, functionally impaired adults or those living with dementia to apply. The following licenses are accepted:
- Licensed Nurse
- Licensed Social Worker
- Licensed Psychologist
- Licensed or Certified Recreational or Physical Therapist
Core Responsibilities
Therapeutic Programming
● Design, implement, and evaluate therapeutic activities that support cognitive function, social connection, emotional health, and physical well-being
● Lead group and individual programming in alignment with dementia care best practices
● Adapt activities for a range of cognitive and physical abilities, ensuring inclusive participation
● Partner with direct care staff to embed therapeutic routines throughout the day
● Implement strategies to support individualized and personalized care for each participant.
Grant-Aligned Program Development
● Support special initiatives and enhanced programming that promotes participant autonomy and joy
● Develop and oversee activity budgets, ensuring responsible purchasing and inventory management for all therapeutic supplies
● Research and introduce innovative, evidence-based interventions into service delivery.
Participant & Family Support
● Conduct intake assessments to determine program eligibility of participants and identify participant interests, emotional needs, and engagement styles
● Collaborate with the Director and Direct Care team to develop individualized engagement plans and update them quarterly or as needed
● Provide ongoing emotional and psychosocial support to participants
● Assist caregivers with education, support, and connection to outside resources
● Maintain accurate, confidential records and contribute to compliance reports Team Support & Education
● Provide training and mentorship to direct care staff on dementia engagement strategies
● Collaborate with the Director on annual training plans for staff development
● Participate in team meetings and interdisciplinary planning for participant success
● Contribute to a strong team culture by stepping in where needed and supporting your fellow staff members
Salary & Benefits
● Full-Time Salary: $48,000 - $51,000 annually
● Monday–Friday schedule (9:00 AM–6:00 PM)
