- Coordinates and processes home health and hospice referrals from referral through transition to service.
- Provides exceptional customer service and communication to referral sources, providers, patients, families, and internal staff.
- Maintains accurate referral, insurance, physician, and transition-of-care documentation within the electronic medical record.
- Collaborates with clinical managers, scheduling staff, billing, hospitals, and referral sources to support timely initiation of services.
- Monitors referral and patient transition status, including hospitalizations, facility admissions, and discharge readiness, to facilitate continuity of care and timely start of home health or hospice services.
- Supports regulatory compliance through accurate documentation management and coordination of required referral processes.
- High School diploma or equivalent required.
- Successful completion of a Medical Assistant program or similar healthcare support training preferred
- Medical terminology coursework preferred.
- One to three years of experience in a healthcare office, referral, scheduling, referral coordination, customer service, or related healthcare support role preferred.
- Experience with electronic medical records and healthcare documentation preferred.
- Experience in home health, hospice, hospital discharge planning, insurance authorization, or care coordination preferred.
- Experience managing multiple priorities in a fast-paced healthcare environment preferred.
- Exceptional customer service and interpersonal communication skills.
- Strong organizational skills and attention to detail.
- Ability to prioritize and manage multiple referrals, calls, and requests simultaneously.
- Ability to communicate professionally and compassionately with patients, families, providers, referral sources, and interdisciplinary staff.
- Strong computer skills, including proficiency with electronic medical records, Microsoft Office applications, and various web-based platforms.
- Ability to work independently and collaboratively within a team environment.
- Ability to maintain professionalism and composure in high-pressure or emotionally sensitive situations.
- Knowledge of medical terminology and healthcare documentation practices preferred.
- Demonstrates proficiency in reasoning, reading, writing, speaking, and problem-solving skills necessary to perform job responsibilities effectively.
Salary : $2,500 - $5,000
