- Integrate health promotion, screening, and brief intervention services into Emergency Department patient care.
- Identify patients with substance use, behavioral health, or social determinants of health needs and initiate appropriate interventions.
- Coordinate referrals and follow-up services with internal teams and community-based providers.
- Maintain accurate, timely, and confidential documentation supporting Project ASSERT outcomes.
- Collaborate with Emergency Department clinicians and support staff to support patient-centered care.
- Bachelor’s degree preferred, or equivalent combination of education and relevant experience.
- Three (3) or more years of experience in one or more of the following:
- Community or population-based health screening
- Primary care or ambulatory care settings with significant patient interaction
- Substance use treatment, intake, or behavioral health environments
- Familiarity with community-based resources preferred.
- Knowledge of substance use disorders, behavioral health screening, motivational interviewing, and harm reduction.
- Strong interpersonal and interviewing skills.
- Ability to communicate clearly, professionally, and empathetically.
- Accurate data collection and documentation skills.
- Proficiency with electronic documentation systems.
- Cultural competence when working with diverse populations.
Salary : $2,500 - $5,000
