Key Responsibilities
- Provide direct personal care to hospice patients in home settings
- Assist with activities of daily living (ADLs) including bathing, grooming, oral care, dressing, and toileting
- Support patient comfort and safety, including proper use of medical equipment (oxygen, bed rails, etc.)
- Perform skin care and positioning to prevent breakdown and promote comfort
- Assist with meal preparation, feeding, and light household tasks as assigned
- Take and document vital signs (temperature, pulse, respirations, blood pressure) as directed
- Observe and report changes in patient condition to the RN Case Manager
- Provide emotional support and companionship to patients and families
- Offer respite care for caregivers when needed
- Maintain accurate, timely documentation in compliance with hospice standards
- Minimum 16 years of age
- Active State Tested Nursing Assistant (STNA) / Certified Nursing Assistant (CNA) certification
- Listed on the state nurse aide registry in good standing
- Current BLS
- Passionate care to terminally ill patients
- Strong communication and observational skills
- Prior experience in hospice, home health, or long-term care
- Experience supporting patients with chronic or terminal conditions
