- Provide personal care and support based on each patient’s individualized plan of care
- Assist with bathing, grooming, dressing, oral care, and other daily living needs
- Ensure patient safety and proper use of equipment
- Prepare nutritious meals and assist with feeding as assigned
- Take and record vital signs when ordered and competency is demonstrated
- Monitor skin integrity and help prevent breakdown
- Report changes in patient condition to the Case Manager
- Offer respite and emotional support to families and caregivers
- At least 16 years of age
- State-certified nurse aide
- Current CPR certification
- Clear listing on the state aide registry
- Understanding of hospice philosophy and comfort working with terminally ill patients
- Compassion, reliability, and strong communication skills
